Asthma, Diabeties and Other Disease Management

Our practice nurses hold regular asthma and diabetes clinics. We offer all our diabetic and asthmatic patients at least one annual assessment with the practice nurse or doctor.

The practice nurses also offer regular blood tests and assessments for patients with a number of other long-term conditions such as heart disease or epilepsy or who are taking certain medications.

Asthma Clinc

The Practice Nursing Team are able to assist in the diagnosis of asthma and are also able to advise on how to manage this with lifestyle choices and use of inhalers best suited to the individual. We like to review everybody at least once a year, however if there is a change in your symptoms before your next review, we are very happy to see you sooner. COPD Clinc (Chronic Obstructive Pulmonary Disease)

The Practice Nursing Team are able to assist in the diagnosis of this condition and are also able to advise on the best way to manage it with lifestyle choices and possible use of inhalers best suited to the individual. We like to review everybody at least once a year, when we perform spirometry (a detailed measurement of your lung function). On the day of this review we would prefer, if possible for you NOT to have used your inhalers, as this will enable us to get a more accurate reading. CHD Clinc (Coronary Heart Disease)

This review is performed by our Healthcare Assistant, and requires a fasting blood test.  Your blood test results will be reviewed by your doctor.  If there is a change in your symptoms before your next review is due, we are very happy to see you sooner. Diabetic Clinic

Qualified Practice Nurses are able to advise patients who are newly diagnosed with diabetes about the condition, healthy lifestyle choices and their medications. The Practice Nurse is also able to refer you to the dietician, retinopathy screening (eye clinic) and the diabetic centre at Worthing Hospital. The Practice Nursing Team like to review all patients with diabetes twice a year.

Before your review you will need to have a fasting blood test 2 weeks before your appointment. Having your blood test taken 2 weeks before will allow time for the results to be entered into your records before your appointment. Hypertension Clinic

If you suffer from hypertension our Healthcare Assistant will monitor your blood pressure twice a year. A blood test and lifestyle review will be performed once a year.
